Have you ever wondered why so many golfers have bad backs? Golf is one of America's favorite pastimes. I would like to share a story with you about a patient who came into my office with a rather typical problem that might sound familiar to a lot of you. Mr. Jones is about 72 years old, he is retired salesman and his general state of health is fair. He is slightly obese; he is also suffering from mild arthritis. When Mr. Jones first came into our office he stated that he typically gravitated toward traditional western medicine and stated that his wife made him come to our office for an evaluation.

When he first arrived in our office, he stated that was unable to play golf because of his back pain and sciatica. He wasn't able to swing the club and walking even from the golf cart to the green, he had to use his golf club as a cane! By the way, this typically is what motivates most golfers to get treatment-when they can't play golf anymore. Mr. Jones further stated that he could not sleep at night because of the pain and it was impacting his life where he couldn't go to the mall with his wife and walk around due to the pain and it was getting very frustrating for him. He stated "he had went to his medical doctor who gave him muscle relaxants and pain pills and he stated that the pain pills temporarily helped, but when they wore off, he was in more pain than ever". Mr. Jones further stated that he had not been to a chiropractor before. When he first came to our office, he had mixed reviews from some of his friends, some got excellent results and some other people did not get results. He also stated that his medical doctor referred him to a neurologist who performed many tests and told him to do physical therapy which he did, with no relief. The neurologist was also talking about surgery and he was not thrilled with that recommendation so he and his wife decided to seek a different type of care.

He stated that he expected chiropractic care to help him to play a round of golf again. We evaluated Mr. Jones and after a complete neurological and orthopedic evaluation, determined that he had a pinched nerve that was causing his discomfort. He would require a series of conservative treatments consisting of spinal manipulative therapy together with Class IV laser therapy. He was also advised to use ice. Initially his treatment program was three time a week for approximately four weeks.

After the first visit Mr. Jones stated that he did feel better and after his fifth or sixth visit he had almost no pain. He stated that at that point he was able to get off all his pain medications and muscle relaxants. After his first week of care, when the pain was considerably less, we began a light stretching exercise routine followed by isometric exercises and then progressed to resistance training, specifically targeting golf. During Mr. Jones second week of treatment, we agreed that that he could begin to swing his club slowly in the yard and if that felt comfortable he could go to the driving range and hit some balls. He felt great about this as he was anxious to get back on the course. After three weeks Mr. Jones was able to play golf without pain and his sleeping pattern was back to normal and he was able to go for that walk in the mall with his spouse, which made her happy.

One of the most common things I hear from patients is "why did I wait so long?" Mr. Jones was surprised and questioned why his medical doctor didn't refer him here quicker. I told Mr. Jones that some allopaths are more supportive than others in referring to chiropractic physicians. Dislocation of the Shoulder

A joint dislocation occurs when the two joint surfaces, which normally sit in intimate contact with each other, are wrenched away from each other to lie apart without any relationship. Joints have a surrounding ligamentous bag called a joint capsule and this can be typically injured as the surfaces force their way past each other. The surfaces of the joints themselves can be damaged as they hit each other on the way to becoming dislocated. Other injuries which can occur include damage to the local nerves and ligaments.

Of all joint dislocations, shoulder dislocations are the most common, making up almost half the total number of this kind of joint injury. An anterior dislocation, with the head of humerus coming off the shoulder socket to the front, is the most common form of this condition. The most usual position for the shoulder to dislocate in is when there is a force applied to the back of the arm with the arm in an outwardly rotated, extended and abducted position. Less commonly a blow to the back of the arm might do it, or a fall on the hand or just moving the arm forcefully outwards and rotating it externally.

A posterior dislocation is uncommon and secondary to a stress on the arm when it is inwards across the body and inwardly rotated, with the large back and chest muscles sometimes pulling the joint out of its socket. This can occur if someone is electrocuted or if they have epileptic seizures, both of which can cause muscle spasms. The shoulder can dislocate downwards if there is a very forceful movement of the shoulder outwards and sideways, with the joint being levered out over part of the scapula above. This sort of dislocation should be closely monitored as complications of the injury are common with nerve damage, blood vessels injury and rotator cuff tears.

There may be no trauma in some cases of shoulder dislocation and instability of the shoulder may occur in all joint directions, typical presenting in patients who have hypermobile joints. This condition is called multidirectional instability and tends to happen in both shoulders, run in the family and be in younger people under thirty. A joint subluxation is often the start of these problems, where the joint slips partly off its partner to an amount and then clicks back into place. An ability to voluntarily dislocate the shoulder can occur, perhaps related to psychiatric difficulties in this group of people.

As the time increases the muscle spasm becomes more severe, making reduction more difficult. An old technique is to place the foot in the armpit to provide stability and pull the arm lengthways until the joint goes back into place. Less traumatically the upper arm can be moved away from the body and the head of the humerus pushed forwards. Once the shoulder reaches 90 degrees the arm is turned outwards and traction applied.

A significant part of a shoulder dislocation is pain and doctors have many ways of ensure the best pain relief and make the reduction process as easy as it can be. Bones remodel to reinforce themselves when they are subjected to loads involving tension or compression, with minor damage of the bone occurring due to the stresses. If the remodelling process gets behind as the microscopic bone damage occurs then a fracture can result. The most common occurrence is for the person to have significantly increased their activities recently.

The risk factors include an increase in the frequency of the applied stresses, an increase in the intensity of those stresses or a change in the area to which the stresses are being applied. If the surface area of the bone to which the stress is applied is reduced then the absolute stresses through those bony areas increase, or the load may be absolutely increased. Running and jumping are examples of more high risk activities, as may be changes in performance technique or in the nature of the surface exercised upon.

Additional factors could be risk factors such as reduced bone density, dietary changes, weakness or other mechanical factors as the other factors are all mostly presumed to be the key ones. Scientific research has indicated being female, having a low body weight, poor diet and many other factors may be important. Female runners are particularly at risk, with reduced caloric intake, disturbances in menstrual cycle and lower bone density presenting in such athletes and others who require a low body weight such as ballet dancers.

A stress fracture typically comes on without much warning and often without severe symptoms, during an activity of repeated limb loading and without trauma. Resting will usually abolish the pain which will re-appear on performance of the weight bearing activity again. Tenderness and swelling may be apparent locally around the fracture site but it may be two to four weeks before a fracture can be discernible on x-ray. Bone scanning may detect fractures much earlier, within 72 hours of the incident, but are less clear as to the exact cause.

The usual management of stress fractures is conservative care, with the simplest and often the most effective method being a reduction in the responsible activity for 4 to 6 weeks. If there is a significant degree of pain on weight bearing then they can be placed in a brace, a rigid walking boot or a below knee cast, with crutch use as required. Orthoses in the shoes have been studied and found to allow a reduction in fracture incidence of a certain amount, with shock absorbing insoles having less clear benefits but potential.

Typical healing of stress fractures is uncomplicated but there is a risk of the fracture suffering from poor healing or non-union in particular bodily regions. The fifth and second metatarsals can suffer from delayed or lack of healing at their bases and as such should be reviewed in case more controlled immobilisation or surgical intervention is required. - 32188

Amputation of The Lower Limb - Part Two

As peripheral vascular disease is the overwhelmingly frequent presentation there are seldom difficulties in establishing a diagnosis and treatment history. As the smaller blood vessels become gradually obstructed gangrene can develop in the toes and the areas which suffer pressure in the foot can develop ulcers. Bacterial invasion can follow with soft tissue infections and progression to infections in the bone. Persistent attempts at treatment can lead to a long period of minor amputations and other surgery which can leave a patient in pain and without useful function.

In cases where the amputation is due to injury or accident it is called traumatic amputation and may be related to severe fractures which are compound and the leg has badly damaged arteries and nerves. The best choice here might be a primary amputation versus attempted saving of the leg. Salvage attempts for the leg can result in the leg being painful and poor in function, limiting progress and encouraging depression. Once the injury has occurred a realistic assessment needs to be made as to the potential for the limb to function well and to decide to amputate should that lead to faster functional return and pain relief.

Keeping the length of the leg and achieving maximum functional independence is the main goal of the amputation process. Because the amputation cannot be undone the surgical decision must be a good one and the only obstacles then are the medical status of the patient in terms of coping with an operation. The abnormality of the limb has a medically negative effect on the overall health status of the patient and its removal may not only save the patient's life but allow them to restore their healthy status. Pre-operative preparation for the operation should prepare for their life after operation and include assessment by a social worker, psychologist and physiotherapist.

Surgical management of severe leg trauma has shown significant advances in the ability to perform microsurgery to the vascular structures, advanced fixation of fractures and techniques to promote revascularisation of tissues. Amputation may then be viewed as a failure if these techniques cannot save the limb, but viewing it as a reconstructive process is more positive, allowing an increase in useful functional capacity. Techniques of amputation have seen much less development and patients still consult with difficulties such as persisting pain, swelling, limited use of the prosthetic limb and feelings of instability.

Surgical technique is to maintain the length of the skin so that it can be folded over the stump and not be under any tension, the muscle is positioned over the cut bone end and opposing muscle groups may be stitched together and the nerves are cut when they are under tension and away from the bone ends so that they are in a non tension environment. One of the general rules for the length of an amputated limb is to allow 2.5cm of length for every 30cm of overall height. Once the operation is over the wound will have the chosen dressing applied and the patient be given post-operative painkillers.

After the immediate period post-operatively the patient will be assessed and treated by a physiotherapist who will review their respiratory condition, teach correct positioning of the remaining limb, encourage appropriate exercises, practice transfers and progress to walking with an aid if possible. Around the two week point the physiotherapist may progress to exercise for the affected extremity and start with a desensitisation programme for the operated part. This involves reducing the tenderness of the limb end so that it can cope with the pressures and stresses of wearing a prosthesis and weight bearing.

The planning and development for the provision of a prosthesis can be started around the six week point as the operated area will have settled to some degree, remembering that some patients will never be able to use a prosthesis well due to mental inability, overall weakness and poor standing balance. Many complications can potentially interfere with the patient working towards their maximum capacity. Vascular disease means potentially poor wound healing or breakdown and poor skin condition and there may be local swelling, joint stiffness, pain problems and phantom sensations. - 32188

When a tear takes place in the outer ring allowing the soft tissue of a disc to bulge out, it is known as a herniated or slipped disc. Inflammation may result in the area as well as pain. Having a job that requires prolonged sitting or lifting can cause this condition. Traumatic injury to the discs in the back is mostly caused by not lifting items correctly. Back fatigue or chronic pain is a sign of the normal wear and tear on the back that increases the likelihood of injuring a disc. Rounding of the spine causes an uneven distribution of pressure whereas a straight spine equalizes internal pressure.

Diagnosis of a herniated disc can be done by a doctor using the history, phyical exam and symptoms. Further testing may be needed to rule out other possibilities such as spondylolisthesis, tumors, degeneration, metastases and possible space-occupying lesions.

Testing may include CAT scan, x-ray, MRI, EMG/NCS and myelogram.

The average healing time for this condition, without surgery, is six weeks.

If pain persists, anti-inflammatory treatment may be prescribed. They should not be used for long term treatment. Steroid injections into the back can also be an option because of the short term pain relief. Physical therapy may also be recommended in combination with other treatments.

Other treatment options are chiropractor visits, weight loss, massage therapy, physical therapy or bed rest.

Lower Limb Amputation - Part Two

Diagnosis is relatively straightforward as most patients will have peripheral vascular disease and will have had considerable treatment for it already. As the small blood vessels block off gradually the toes can develop gangrene and ulcers on the pressure areas, which permits bacterial invasion leading eventually to bony infection. If treatment is incremental there may be a long period of minor amputations and other operations, all the time the patient being unable to walk due to pain in the limb. They may use wheelchair due to the pain and energy requirement of ambulation.

In accidents and injuries the amputation may be traumatic or involve a compound fracture severe enough to damage the nerves and blood vessels beyond salvaging. Here an amputation may be the best choice as against long term efforts at salvaging the damaged limb. An attempt at salvaging the limb may result in a non-functioning leg which is painful, leading to mood changes such as depression and limited activity. A careful assessment of the potential to save the limb needs to be made after the injury to avoid a lengthy and painful treatment period when amputation might deliver good function much sooner.

Now, just how do these back injuries really come about? The first thing we need to is understand the core parts of the spinal column. It is comprised of 24 moving spinal bones called vertebrae. These vertebrae are stacked one on top of each other just like small blocks and sitting in between each one are gel like shock absorbers, or discs.

Connecting the vertebrae, holding them in alignment, almost rubber bands are ligaments. Muscles are also attached to the vertebrae by way of tendons. As your muscles contract they are responsible for moving the vertebrae which in turn moves your torso and upper body.

Normally, the average human lower body weighs about 105 pounds. Due to earth's gravity, a 10 pound object added to the weight of the human lower body creates approximately 1150 pounds of pressure on your lower back. If you are 25 pounds excessive weight, which seems to be very common, you are placing another 250 pounds of pressure on your lower back every time you bend over or try to pick something up. This is a really surprising or shocking statistic.

So what's the big deal all about? Every time you lift, pressure begins in the middle of your disc and that pressure will push out from the center of the disc toward the outside of the disc. If there is too much pressure, the outer disc fibers will begin to weaken and tear. If enough outer disc fibers are damaged from lifting excessively or even lifting wrong, the outer walls of the disc become weakened to where literally the center core can bulge through the outer fibers and pinch or put pressure on your spinal cord or spinal nerves. That is what happens when a person has a "slipped" or bulging disc.

The problem is, the outer spinal disc fibers in your lower back start tearing anytime you create 1500 lbs. pressure on your lower back area.

Most back pain sufferers feel that one particlular event or single incident started or caused their lower back pain or back injury. The reality of the matter is, less than 5 percent of all back problems or injuries can be blamed on one single injury or incident.

About ninety-five percent of back problems seem to build up over time. Big problems come about gradually over long periods of time until a singular simple small movement causes you to "throw your back out" and cause alot of pain. We all know the "straw that broke the camels back" story.
